We are actively enrolling patients in an exciting research study for patients with Type 2 Diabetes Mellitus. You may be eligible to participate if you meet the following criteria:

• 18 years old or older
• Have an A1C between 7 and 10.5%
• Have a BMI of 25 or greater
• Currently being treated with Metformin by itself or in combination with a SGLT2 inhibitor such as Jardiance, Farxiga, Invokana or Steglatro

Eligible patients who enroll in the study will receive free study medication, a free eye exam, free diabetes testing supplies, and reimbursement for time and travel.
