Chronic Heart Failure

You may be eligibe to participate if you;

• are 18 years of age or older
• have been diagnosed with chronic heart failure
• have not had a serious heart event (such as a heart attack, stroke, or major heart surgery) within the last 90 days

Type 2 Diabetes with Moderate Chronic Kidney Disease

You may be eligible to participate if you:

• Are 18 years of age or older
• Are a Type II diabetic with moderate renal impairment
• Have A1c levels between 7 - 10 %

Type 2 Diabetes with Severe Chronic Kidney Disease

You may be eligible to participate in the SOTA-CKD4 study if you:

• Are diagnosed with Type 2 Diabetes
• Are 18 years of age or older
• Have HbA1c levels between 7-11%
• Have been diagnosed with kidney disease

Type 1 Diabetes

You may be eligibile to participate if you:

• Are between the ages of 18 and 75 years of age
• Have a confirmed diagnosis of Type 1 diabetes 
• Have an HbA1c < 11%
• Be taking a short-acting and long-acting Insulin medication taht your doctor prescribed for at least 6 months
